More than 670 people have been infected in Utah\u2019s measles outbreak reported on this week in Wired\u2019s Big Story, and the cases that should alarm us most are the ones no vaccine could have reached. Twenty-three infants, a dozen pregnant women \u2014 one of whom passed the virus to her newborn \u2014 and a growing number of immunocompromised young adults. For each of them, the measles-mumps-rubella shot is not an option. It is a live-virus vaccine, contraindicated in precisely the patients most likely to die if infected. What can stand between these Americans and a preventable tragedy is not a vaccine. It is an antibody.<\/p>\n

When I tell my former colleagues in Congress that I now lead a coalition devoted to monoclonal antibodies for infectious disease, the most common follow-up is some version of: Aren\u2019t those mostly for cancer? The question itself is the problem. It explains, more than any committee report could, why a class of medicines that has reshaped modern infectious disease has spent the better part of a decade fighting for the clinical and policy attention it deserves.<\/p>\n

Monoclonal antibodies, or mAbs, are laboratory-engineered proteins that mimic the body\u2019s own immune defenders. Where a vaccine teaches the immune system to build its own antibodies over time, a mAb skips that process and delivers a finished, ready-to-deploy product. For a healthy adult, that distinction may sound academic. For an infant whose immune system has never encountered RSV, a transplant patient on immunosuppression or a leukemia patient depleted of B-cells, it is the difference between protection and exposure.<\/p>\n

Consider what mAbs have already delivered. Nirsevimab, a single-dose RSV antibody given to infants entering their first respiratory season, has cut RSV hospitalizations by more than 70% and ICU admissions by up to 89% in real-world studies. Two mAb cocktails, Inmazeb and Ebanga, transformed Ebola from a near-certain death sentence into a treatable infection, and two others sit in the Strategic National Stockpile as biodefense countermeasures against weaponized anthrax. Each is an FDA-approved product. None received the public attention of a vaccine campaign. All quietly save American lives.<\/p>\n

The Utah outbreak is part of a string of large measles flare-ups that pushed national case counts in 2025 to 2,242, the highest since the disease was declared eliminated in 2000. Because the MMR vaccine contains a live, weakened virus, it cannot safely be given to infants under one, to pregnant women or to people with suppressed immune systems \u2014 the very groups for whom measles is most dangerous. Pregnant women face significantly higher risk of dying from measles complications than healthy adults. An infant infected before her first birthday faces heightened risk of subacute sclerosing panencephalitis, a fatal brain disorder that can surface years after the original infection.<\/p>\n

When patients in these groups are exposed, antibodies are the only protection medicine can offer. Earlier this year, Utah officials reported that 130 babies too young to be vaccinated received antibody injections after exposures in pediatricians\u2019 waiting rooms. Those doses were immune globulin, pooled from donors, polyclonal and chronically scarce. The underlying principle is the same one that drives every monoclonal antibody in this field: when a vaccine cannot prepare the immune system in time, a finished antibody can stand in its place. A purpose-built monoclonal would do that job with more potency, more consistency, and at far greater scale.<\/p>\n

Science has done its part. The deployment infrastructure has not.<\/p>\n<\/blockquote>\n

The same protection gap persists for millions of immunocompromised Americans at risk from COVID-19. For patients on B-cell depleting therapies, awaiting solid organ transplants, or living with hematologic malignancies, vaccines provide inadequate or no protection. The FDA authorized pemivibart (Pemgarda) under Emergency Use Authorization specifically because there is \u201cno adequate, approved, and available alternative\u201d for patients \u201cunlikely to mount an adequate immune response to COVID-19 vaccination.\u201d Yet pharmacy access, coverage codes, co-pay pathways and clinician awareness all remain critically underbuilt. Closing that delivery gap is not a scientific problem. It is a policy problem.<\/p>\n

The science is moving fast. Recent peer-reviewed work has shown engineered antibodies that retain potent neutralization across every SARS-CoV-2 variant tested, with up to 100-fold improved breadth over first-generation mAbs, offering a potential path to durable protection that does not require reformulation with every new wave. These antibodies also represent a regulatory approach fundamentally different from a seasonal vaccine, one where variant coverage updates can be handled through laboratory data rather than a new Phase 3 trial. The annual scramble to match a vaccine to a circulating strain is expensive, time-consuming, and inherently a step behind the virus. A broadly neutralizing antibody platform changes that dynamic.<\/p>\n

The potential is too consequential to treat this as a second-tier policy concern. Science has done its part. The deployment infrastructure has not. Closing that gap before the next variant, the next outbreak, or the next surge can preempt the drastic costs of being underprepared.<\/p>\n
